Our Lady of Light Parish St. Catherine of Sienna and St. Pascal Baylon churches

To All Parish Organizations and Members:

Our Lady of Light Parish is asking all parish organizations in the church who use the church facilities for meetings or other than liturgical purposes to fill out one of the forms listed below. These forms are needed so that we know who is on premises and using the facilities at all times. This information is needed for insurance purposes. They are also going to be used to ensure that the dates and times of events are on the parish calendar and there are no conflicts. All forms will be approved by the PPC, Finance Council and when necessary by Fr. Smith.

If any member of a parish organization wishes to use any part of the church facilities, or plan a fundraising activity the following forms must be filled out. 

  1. a Facility Usage Agreement form to be used by a nonprofit organization with a 501c and insurance; (this form is for non parish member),
  2. a Fundraising Financial Report (fundraising in or out of the parish),
  3. or a Non-Fundraising form (for meeting usage purposes).

Forms must be Signed by the PPC and Financial Council and then submitted to Mrs. Mary Harris for availability and approval. If the dates are approved, Millicent Platts will send a copy of the approved form to the person who submitted the form. Forms may be obtained from the office or downloaded below.

All fundraising activity should not only support the organization giving the fundraising. Each organization should also support the church with a donation thirty days after each event.
